I am pretty sure RSSBandit has this feature, and since I have not used that reader in at least a year, I am guessing it has had it for quite a while.

Guess it is limited to sites that support the proper APIs. If the blog post has the wfw:commentRss ewlement RSS Bandit will go and get the comments from that feed and display the items from it as a thread under the blog post.
